AI Voice Recorder for Quality Managers: Evidence Before CAPA Closure

A corrective-action meeting can sound decisive while producing no evidence that the cause was addressed or recurrence reduced.

This workflow links every audit statement, non-conformance and CAPA action to objective evidence and a defined effectiveness check.

The evidence ladder

The defensible finding

Write three elements:

  1. Requirement: exact standard, procedure or contractual criterion.
  2. Evidence: verified sample, observation or record.
  3. Gap: how the evidence fails to meet the requirement.

Avoid vague language such as “staff are not following procedure” unless the evidence proves that precise conclusion.

Correction is not corrective action

  • R — Response: attributed explanation.
  • A — Artefact: record, sample or document reference.
  • C — Check: verification performed.
  • E — Evaluation: conclusion or evidence still required.

The CAPA workflow

  1. Define audit scope and criteria.
  2. Confirm recording permission.
  3. Generate and correct the interview transcript.
  4. Collect independent objective evidence.
  5. Write the requirement-evidence-gap finding.
  6. Apply containment and correction where needed.
  7. Investigate process, system, workload and organisational causes.
  8. Approve action with owner, deadline and intended result.
  9. Verify implementation.
  10. Run a pre-defined effectiveness test.
  11. Close only through the controlled QMS.

Plan the effectiveness test before action

Define the later evidence at approval time:

  • sample size and period
  • performance or defect measure
  • process observation
  • recurrence threshold
  • unintended-effect check
  • person authorised to close

“No complaints received” may be weak evidence when the problem is not visible to customers.

Supplier quality

Keep supplier explanation, evidence promised, containment agreed, formal response and disposition authority separate. Confirm material commitments through the approved supplier-quality process.
