A structured spoken inspection note
- State the inspection reference and date.
- Identify the exact location, asset, batch or process.
- Read the requirement or checklist item.
- Describe the observation without explaining it.
- State evidence and measurement references.
- Record any immediate safety or containment action already completed.
- Label possible cause or consequence as interpretation.
- State who must review or classify the finding.
- Identify the required formal record.
Verify transcript details that can change the finding
Check manually:
- clause, asset and drawing numbers;
- decimal points, signs and units;
- directions and locations;
- batch, serial and sample identifiers;
- speaker attribution;
- negative wording such as “not isolated”;
- dates and times; and
- whether the statement was measured, estimated or reported by another person.
From finding to corrective action
A correction fixes the observed problem. Corrective action addresses the cause to reduce recurrence. Keep them distinct.
