Why shift handover is safety-critical

HSE describes the goal of handover as accurate, reliable communication of task-relevant information so work continues safely and effectively. It identifies three elements:

  1. preparation by outgoing personnel;
  2. information exchange between outgoing and incoming personnel; and
  3. cross-checking by incoming personnel as responsibility transfers.

A recording is support—not the handover itself

A one-way audio file cannot ask questions, inspect the plant, challenge an assumption or confirm understanding. Critical handovers should use the organisation’s required face-to-face or live two-way process, supported by written logs, displays, permits and records.

A recording may help:

  • draft the outgoing summary;
  • preserve a permitted live briefing for verification;
  • support remote participants where the process allows;
  • capture agreed actions; or
  • review recurring communication weaknesses.

It must not delay the live transfer of control.

Prepare the outgoing handover

Before the incoming shift arrives, review the authoritative systems and prepare the current state.

  • operating status and production or service position;
  • safety, security, welfare or environmental risks;
  • abnormal conditions, alarms and deviations;
  • equipment unavailable, isolated or overridden;
  • permits to work and maintenance in progress;
  • temporary repairs, controls and compensating measures;
  • work completed and verification status;
  • work in progress and safe stopping point;
  • people, parts, approvals or information awaited;
  • incidents, near misses and investigations;
  • first priorities and timed checks for the incoming shift; and
  • escalation triggers and contacts.

Use a fixed risk-first sequence

  • the action already taken; and
  • the authoritative log, permit or work order.

Use read-back and cross-checking

The incoming person should summarise critical points back in their own words and check them against:

  • control-room displays and alarms;
  • operational logs;
  • permits and isolation records;
  • work orders and maintenance status;
  • physical inspections where required;
  • staffing and competency arrangements; and
  • emergency or escalation instructions.

Any discrepancy should be resolved before responsibility is treated as fully transferred.

AI transcript risks in a handover

Errors can be safety-critical. Check manually:

  • asset and tag numbers;
  • “open” versus “closed” and “on” versus “off”;
  • negative language such as “not isolated”;
  • readings, units and decimal points;
  • times, dates and shift references;
  • permit status;
  • speaker attribution; and
  • conditional or provisional instructions.

Do not use an AI summary as the sole source for operating a safety-critical system.

After the live handover

  1. Complete the required log and transfer-of-responsibility process.
  2. Confirm urgent tasks and timed checks in the official system.
  3. Link permits, work orders, incidents and temporary controls.
  4. Correct any permitted transcript against the live discussion and records.
  5. Record discrepancies or missed information for improvement.
  6. Restrict access to operational and personal information.
  7. Delete temporary audio according to the organisation’s retention process.

Do not wait for AI processing

Emergency, safety, security, safeguarding and environmental information must be communicated and escalated immediately through the authorised process. Recording and transcription are administrative support, not an emergency channel.
