A case note should help an authorised colleague understand the current position without replaying the entire conversation. That makes it fundamentally different from a transcript. The transcript preserves speech; the case note preserves source, status, professional reasoning, risk and next action.
Recording can reduce memory gaps, but it can also introduce new errors when speaker labels, dates or uncertainty are wrong. The safest workflow treats AI output as a drafting aid and requires a competent person to approve the final record.
Use five status labels throughout the note
The most important improvement is to show what kind of information each sentence contains.

AI summaries often merge these categories into a single smooth narrative. Restore the labels before filing the note.
Move through a source-to-record funnel
Stage 1: capture only when the process permits it
Define why the conversation is being recorded, what the person will be told, who may access the audio and when it will be deleted. Sensitive cases may require an approved specialist system rather than a general recorder.
Stage 2: correct the source
Check names, dates, reference numbers, speaker attribution, negative wording and any specialist terminology. An incorrect transcript should not be summarised until the material error is fixed.
Stage 3: extract the case-changing information
Identify only what changes understanding, risk, eligibility, service delivery or action. Greetings, repeated history and unrelated private details rarely belong in the final record.
Stage 4: apply professional judgement
Explain why the information matters, what remains uncertain and what policy or professional process applies. Do not let AI supply a missing assessment.
Stage 5: file the approved note
Move the final record into the authorised case system with the correct author, date and status. The recorder app should not become a parallel case file.
A case-note structure another professional can use
- Contact context: date, method, participants and purpose.
- Current position: concise description of the issue.
- Information reported: source-attributed statements.
- Evidence observed or verified: documents, measurements or checks.
- Professional assessment: reasoning, uncertainty and relevant context.
- Risk: urgency, immediate controls and escalation.
- Decision: outcome, authority and rationale.
- Actions: owner, deadline and dependency.
- Communication: what was explained, sent or confirmed.
- Record status: author, review and filing location.
Before-and-after example
Weak AI draft: “The client has not received support and the team agreed this was urgent.”
Controlled case note: “The client reported that no support visit had taken place since 14 July. The rota was not available during the call, so this has not yet been independently verified. I assessed a potential immediate-care risk because the client also reported having no food in the property. The duty manager authorised a welfare check for the same afternoon. Jamie Patel owns the action and will update the case by 5pm.”
The second version shows source, uncertainty, assessment, authority, owner and deadline.
Create a separate risk lane
Urgent information should not wait for routine transcription. Where a conversation indicates immediate safeguarding, clinical, legal, financial or personal-safety risk, follow the organisation’s escalation route first. Document the action afterwards.
The recording may support the chronology, but it must never delay protective action.
Minimise without making the note useless
A proportionate note includes enough detail to explain the decision and next step. It does not reproduce every private detail simply because the transcript captured it.
Remove:
- Unrelated personal conversation.
- Duplicated history already held in the case file.
- Speculation that did not affect the decision.
- Third-party information without a clear purpose.
- Long quotations where an accurate attributed summary is sufficient.
Keep qualifications, disagreement and context that materially affect interpretation.
Handle corrections transparently
Do not silently overwrite a material historical record. Follow the organisation’s amendment process so the original entry, correction, date, author and reason remain understandable. When a person disputes an opinion, identify it as an opinion and record the challenge where relevant.

A strong case note lets another authorised professional answer six questions quickly: what happened, who supplied the information, what is verified, what was assessed, what was decided and what happens next.
If the note cannot answer those questions without opening the raw transcript, it is not finished.
