The STOP note
- S — Station: site, line, machine, product and batch.
- T — Time and symptom: first event, alarm and operating condition.
- O — Operational control: stop, isolation, quarantine or diversion.
- P — Plan: technical owner, next check and records required.
Say what was observed before possible cause. “Alarm E27 displayed” is evidence; “motor failed” is a diagnosis requiring confirmation.
Define the affected-product window
After an unexplained stoppage or process deviation, identify:
- last known good point
- first possible affected time
- product, batch or serial range
- material still in process
- stock already moved or despatched
- checks and samples required
- person authorised to release or reject
A successful restart must not release questionable product automatically.

Record short notes away from machinery where possible. Spell specialist terms and state numbers digit by digit. Never rely on transcription where one wrong word changes a safety or quality decision.
