The safest approach is a staged review that checks the source, the meaning and the destination separately.
Pass one: source accuracy
Compare the transcript with the audio for details that can change the record:
- Names, organisations and speaker labels.
- Dates, times, figures, units and reference numbers.
- Negative words and conditional language.
- Technical, legal, medical or product terminology.
- Quotations and disputed wording.
Mark passages that remain unclear instead of replacing them with plausible text.
Pass two: status and meaning
Label each important item as discussion, proposal, agreement in principle, final decision, action, risk, question or rejected option. AI summaries can compress these distinctions.

Pass three: destination fitness
Ask what the receiving system needs. A CRM note should not contain a complete internal debate. A case record needs source status. Minutes need decisions and actions. A knowledge article requires tested, reusable instructions.
Remove irrelevant personal data, internal commentary and duplicated content before transfer.
Use a risk-based review depth
- Low impact: check identifiers, actions and summary meaning.
- Medium impact: replay every commitment, figure and decision.
- High impact: qualified review, source timestamps, controlled approval and possibly a second reviewer.
The apparent fluency of the transcript should never reduce review depth.
Keep a correction trace
For important records, retain the correction date, reviewer and reason. Distinguish transcript corrections from later changes in the underlying business position.
