A RACI matrix clarifies who is responsible, accountable, consulted and informed for a defined deliverable or decision. Meeting recordings can preserve ownership discussions, but phrases such as “we will handle it” or “finance should look at that” are not sufficient assignments.
Start with work packages, not people
List the decisions, deliverables or controls that need ownership. Keep them at a useful level: large enough to matter, but specific enough that completion can be tested. Do not create a RACI row for every minor task.
Apply each role precisely

Usually each row should have one clearly accountable role. Several responsible people may contribute, but broad “team” ownership often hides the real gap.
Extract candidate ownership from the source
Use the transcript to locate:
- Direct commitments.
- Delegations and approvals.
- Dependencies on specialist input.
- People who challenge or accept a decision.
- Statements that authority is missing.
- Deadlines or review triggers.
Check material wording against the audio. A speaker suggesting a person is not the same as that person accepting responsibility.
Resolve common defects
- No accountable owner.
- Two competing accountable roles.
- A senior person shown as responsible for work performed elsewhere.
- People assigned without authority, capacity or knowledge.
- Consulted lists so broad that decisions cannot progress.
- Informed roles receiving confidential detail they do not need.
Confirm the draft with named owners
- Send the proposed matrix with definitions.
- Ask each accountable person to confirm or correct their role.
- Resolve overlaps through the project or governance owner.
- Check that authority and resources match the assignment.
- Record the approval date and version.
Connect the matrix to real work
Transfer ownership into project plans, role descriptions, approval routes and task systems. A RACI stored separately from the operational workflow quickly becomes obsolete.
Review after change
Update the matrix after restructures, supplier changes, scope changes or repeated ownership failures. Compare actual decisions and actions with the documented roles.
