Policies cannot predict every meeting. An exception process gives staff a controlled route when the normal recording rule does not fit, without turning “urgent” into permission to improvise.
Define what counts as an exception
- A new use case not listed in the approved-use matrix.
- Unexpected sensitive or restricted information.
- A participant objection or accessibility need.
- Failure of the approved device, app or account.
- An urgent operational need requiring a different method.
- A request to retain, share or reuse data beyond the original purpose.
Use a short escalation form
Capture purpose, context, people affected, information type, proposed method, urgency, alternatives, requested duration and named decision maker.
Create decision levels

Allow only time-limited decisions
An exception should state exact boundaries, controls, expiry and review. It must not silently become a permanent new policy.
