Maintenance records fail when they describe activity but not condition. “Checked motor, cleaned area and reset alarm” does not tell the next shift what triggered the alarm, what was ruled out, whether the defect is developing or what operating restriction remains.

  3. Controlled: isolation, monitoring, restriction or temporary measure.
  4. Corrected: authorised repair or adjustment completed.
  5. Verified: test result and closure authority.

Preserve the diagnostic path

When troubleshooting, state why each test was chosen and what it changed. A compact format is:

Hypothesis — evidence expected — test performed — result — hypothesis supported or rejected — next action.

Keep failed attempts and no-fault-found results. They are part of the asset history and may reveal a recurring condition when several work orders are reviewed together.

Distinguish maintenance action from engineering change

Replacing like-for-like components under an approved maintenance instruction is not the same as changing a material, setting, dimension, control logic or protection method. When the proposed action changes the design basis, stop and route it through engineering change control.

A voice note should flag:

  • non-standard parts or substitutions
  • changed setpoints or software
  • removed guards or bypassed interlocks
  • repeated temporary repairs
  • modified lubrication, alignment or torque requirements
  • drawings or procedures that no longer match the equipment

Make preventive maintenance results measurable

“PM completed” gives little evidence that the asset condition was acceptable. Record:

  • task step and acceptance criterion
  • actual observation or measurement
  • adjustment made, if any
  • consumables and quantities used
  • defects raised for follow-up
  • next inspection trigger or interval

Trendable values should be entered in the system fields designed for analysis, not left buried in free text.

Build a reliable shift handover

For open work, the next shift needs an operational picture, not a transcript. Summarise:

Remaining uncertainty What has not been proven

Verify return to service

Before closure, check that guards, tools, isolations, permits, housekeeping and temporary controls are addressed through the approved process. Record the functional test, conditions, result and person authorised to return the asset to service.

Do not use phrases such as “all okay” when the machine was tested only unloaded or for a short duration. State the limits of the verification.
