A field-service report often has to answer three different questions at once: what the customer reported, what the technician actually found, and what the equipment was left capable of doing. When those three are blended together, disputes and repeat visits become far more likely.

Open the visit with a condition snapshot

Before testing or dismantling anything, capture a baseline:

  • recent repairs, software changes or environmental events

This distinction matters when the fault cannot be reproduced or when the repair scope changes during the visit.

Use a diagnostic branch record

A long narrative can hide why a technician chose one test over another. Record the investigation as branches:

  1. Symptom: what must be explained.
  2. Possible causes: credible alternatives, not every imaginable failure.
  3. Discriminating test: what result would support or reject each cause.
  4. Result: source-referenced evidence.
  5. Next branch: why the investigation moved in a particular direction.

For an intermittent fault, also capture the conditions in which it did not occur. Negative evidence helps prevent the next technician from repeating the same checks.

Record configuration before changing it

Many repeat visits happen because the pre-change state was not preserved. Before adjustment, replacement or software work, note:

  • settings, firmware and software versions
  • switch, valve or control positions
  • connector and terminal references
  • network or interface configuration where permitted
  • part numbers and fitted revisions
  • temporary bypasses or previous modifications

After the work, record the new state and the authority for the change. Do not place passwords, access tokens or unnecessary customer data in an audio file.

Control scope changes and customer approval

A voice note can document that a new issue was discussed, but the commercial or safety authorisation must be completed through the approved process. A useful variation record includes:

Original scope — new finding — consequence of not acting — proposed work — price or time impact — person authorised to approve — approval reference — work completed.

Avoid vague phrases such as “customer said go ahead.” Identify the authorised person and the formal approval route.

Capture parts and consumables accurately

Check every part number, quantity, batch, serial number and revision against the label or system record. Speech recognition can easily confuse similar component codes.

For removed parts, note:

  • condition and failure evidence
  • whether retained, returned, quarantined or disposed of
  • warranty or analysis requirement
  • replacement part and revision
  • photograph or return-material reference

Prove the outcome with a before-and-after test

“Working when left” is too weak for a professional handover. State:

  • test performed and acceptance criterion
  • operating conditions during the test
  • actual result and evidence reference
  • duration or number of cycles
  • remaining limitations or unverified conditions
  • whether the original symptom was reproduced and cleared

If the fault was not reproduced, say so plainly. Do not claim a repair is proven merely because no alarm occurred during a short test.

Write the leave-state before leaving site

The final service note should tell the customer and the next technician exactly what state remains:

Use recording safely and lawfully on customer sites

Do not record while driving, climbing, lifting, working near moving machinery or carrying out any task that requires full attention. Obtain permission before recording identifiable people and follow the customer's security rules. Sensitive conversations, screens, documents and personal data should not be captured unless necessary and authorised.
