Mechanical problems rarely arrive as a neat sentence. A machine may vibrate only after warm-up, a valve may stick under one operating condition, or a design review may uncover an interface assumption that never reached the drawing. The record must capture the conditions that make the observation meaningful.

Start with the operating envelope
Before describing a symptom, state the context in which it occurred. A reliable note should identify:
- asset, assembly or drawing reference
- operating mode and duty point
- load, speed, pressure, temperature and relevant environment
- time in service or time since start-up
- lubrication, cooling and utility condition
- recent maintenance, adjustment or design change
- guards, permits and safe state
The same sound, temperature or movement can mean very different things at idle, full load, cold start or steady state.
Describe behaviour without promoting it to a diagnosis
Mechanical troubleshooting improves when observations and explanations remain separate. Use language such as:

This structure prevents an AI summary from converting a possibility into a failure cause.
Capture fault progression, not just the final state
For intermittent or developing faults, record the sequence:
- Initial machine condition.
- Command, load change or environmental trigger.
- First observable response.
- Alarm, measurement or inspection evidence.
- Action taken.
- Immediate result.
- Whether the symptom returned.
Include unsuccessful interventions. If tightening a mount, changing a filter or adjusting a setting did not change the symptom, that negative result narrows the next investigation.
Use a failure-mode note for structured diagnosis
A concise failure-mode note can be organised under:
- Function: what the component or system must do.
- Failure behaviour: how performance differs from the requirement.
- Potential mechanisms: wear, fatigue, contamination, looseness, distortion, cavitation, blockage, thermal growth or control interaction.
- Discriminating evidence: the test that would separate one mechanism from another.
- Immediate risk: safety, containment, secondary damage, production or environmental consequence.
- Next authorised action: inspect, monitor, isolate, repair, redesign or escalate.
Do not use the transcript as the risk assessment or permit. Those controls must remain in their approved systems.
Make design reviews decision-led
Long design-review transcripts are difficult to use unless they are converted into decisions and unresolved interfaces. For each significant issue, capture:
- requirement and source
- assumptions about load cases, materials and service conditions
- interfaces with structures, electrical systems, controls, maintenance and access
- options considered
- manufacturability, inspectability and maintainability implications
- selected direction and authority
- calculation, drawing, test or supplier evidence still required
- date or condition for review
Discussion does not revise the design. Approved changes must reach the controlled model, drawing, bill of materials, specification and verification plan.
Record dimensional and material data carefully
Speech recognition commonly confuses decimal points, units and similar identifiers. Check every dimension, tolerance, fit, thread, material grade, hardness, surface finish, torque, pressure, temperature and drawing revision against the source.
Never rely on a transcript alone for:
- critical tolerances or acceptance limits
- lifting points and load ratings
- pressure-boundary requirements
- fastener grades and tightening values
- weld procedures or inspection criteria
- material substitutions
- safety-factor calculations
Turn commissioning observations into a punch-list entry
When a mechanical commissioning issue is found, the note should be capable of producing a clear punch-list draft:
Asset — location — drawing revision — test stage — expected behaviour — actual behaviour — evidence reference — operational restriction — owner — due date — retest requirement — closure authority.
A statement such as “pump noisy during test” is not actionable. “P-204 cavitation-like noise at 72% speed with suction pressure 0.18 bar below test-plan minimum; test paused pending suction-side inspection” gives the team something to verify.
Protect safety and confidentiality
Do not handle a recorder near rotating equipment, exposed hot surfaces, pressurised systems, lifting operations or any activity requiring full attention. Record only after the task is controlled and from a safe location. Obtain permission where people are identifiable, and avoid including personal information, restricted designs or customer data unless policy allows it.

- Does each design decision identify its approval and downstream document changes?
